measurements (2"x4")??? This is EAPG and Fostoria Glass #1299 Long Buttress c1904. One place mentions this form was advertised as a 'sanitary' toothpick holder.

Fabulous ho2cultcha. I could really do with a toothpick holder, seriously, they get everywhere.
