The Fahys Montauk is the case maker, Fahys is the maker, Montauk is the grade, a 20 year gold filled case.
The watch movement is manufactured by the American Waltham Watch Co.
If I read the serial number on the movement right (it is not real clear when enlarging the image) it is 18413128. If that is correct it is a:
Grade 620
Model 1908
16 size movement
15 jewels
Manufactured about 1912
