It's a Breast Drill used to bore large holes in wood. It uses the old fashioned screw bits like a regular hand brace would use. The carpenter would lean their chest against the bottom plate to provide the force needed to make larger bits take a good bite while drilling.
That one looks like it has sealed gears and so may have more than one gearing.
Shroder and Miller Falls were both big manufacturers.
