I can't nail this one down to any particular maker. The closest I've come is this web page - http://retroartglass.com/product/vintage-crackle-glass-decanter-and-ball-stopper/
that says possibly Italian or by Kanawha Art Glass.
It is 8-1/2 inches tall with a 4 inch diameter base. I'm hoping someone will recognize it.
I finally got a nice cabinet for all my decanters so have been locating them one by one :)
I believe this might be BLENKO -- as they did crackle glass.

That one in the link does NOT have a label, so might want to look to see if you can find one with a label.
