Japanese doll with 3 faces

I'm hoping that someone can help me figure out some information about this doll. It has a porcelain head with three faces and porcelain hands. The clothing is made from silk, and the only information I have is that the doll was purchased in Japan by my great grandfather for my great grandmother.
