Is there anything written/etched into the medallion shape on the front? It's not carnival glass, and gravy wasn't usually served from glass. It's more likely to be a creamer, even then at 5 1/2 inches it's quite tall for a creamer. Our resident glass expert, and most revered, TallCakes may stop by and give you some information. Whatever it is, it's a lovely piece of glass. Welcome to CW from me.
Bonnie you're quick - however I did leave the post half-way thru for a while. Great ID Bonnie - I knew it would be some kind of memorial piece. Can't get much better than Bonnie's ID.
Thanks, Gillian!

"The Avon Fostoria “Mount Vernon” Creamer, also known by some as the Small Pitcher, is a bright cobalt blue and is about 5 and 1/2 inches tall. It is about 4 -1/2 inches across the opening from handle to spout to handle, and 3-1/4 inches across the opening front to back."
