Hi,
You have a very nice Windsor arm chair. And, as you say it's a double spindle back maple dining chair. There are so many Windsor arm chairs out there, I did a google search and found many that looked like yours. Many variants.

'Brief history of the Windsor chair' Sorry about the crafts video, just pause it and continue reading after it.
I can't identify your chair by the numbers you quoted - if there's not a label under the seat it's really difficult to make a manufacturer's id.
All the best, and good luck - and welcome to CW.
We had a dear friend pass. When helping his partner clean the house I noticed a chair with a paper tag attached to the bottom. I have a friend who does reproduction work for museums of Windsors. After lots of calling, emailing, research etc we discovered the chair may have been present at Independence Hall during the signing of the Declaration of Independence! Its currently at Independence Hall in Philadelphia in the signing room! They were thrilled to receive as as we were thrilled to donate it! Your chair looks ALMOST IDENTICAL to the very old one we donated. Our chair was made by Joseph Hensley so you may try researching him first. I have lots more info if interested I guess you can leave a comment or whatever you would like. Good luck!!
Thank you for sharing your story and the research you both have done. As it turns out, there was a very faded "Ethan Allen" painted on the base of the seat. Although the Windsor chair it is not a valueable antique, it is still a very nice chair.
Thanks again.
