Hello I have found this item and am wondering if it is a art nouveau pendant, or is it something else, I don't know what the stone is or the age.

Looks like maybe it opens by pushing in on the little knob & lifting?
Yes it opens cameo , it could be a clasp I've seen lock like this but normally just plain.
I too have seen heart shaped padlocks, usually as clasps on bar gate bracelets, much plainer than this, but still think that is what this has to be, if large enough. The loop mounted at the top would be to attach a safety chain so the heart doesn't fall off altogether.
The stone looks like a blue topaz , I have handled these and they modern ,probably made in China
