Anyone out there know what this thing is all about. Kinda spooky looking and I know nothing about it, but it does have “Italy” engraved on the back.

Beautiful! Looks like a serving spoon, was probably silver plated at one point
Here is a great link to a silver site showing hallmarks from all over the World. This particular link will open to Italian makers of silver and silver-plate / hollowware, so you may find an example of your mark, which is quite worn, but you may recognize the shape, along with the word ITALY

Could it be an "absinthe spoon"?? Just guessing, I truly have no idea whatsoever...? :-)
Thank you for all the feedback. After following your links and doing some digging I think it is a serving spoon that was once silver plated. It would make sense if it was an absinthe spoon with the creepy image of those two kids on it, but I think it’s too big for that. Anyways, thank you for taking the time to check this out and for the info!
