This is a Hopi Katsina doll, which is the Heyheya Katsina. This doll was made sometime in the 1920's in the area of New Mexico/Arizona. It still has remnants of the string around it's neck, which was used to hang it on the wall. It is one piece, carved from a piece of cottonwood. It stands about five inches tall. Most of the mineral paint has faded with age.
