This toothpick holder is 2 1/2" (6.4 cm) tall x 2 5/8" (6.7 cm) wide at the top and 1 5/8" (4.1 cm) wide at the base. The mark on this toothpick holder is inside bottom and on the outside bottom of the piece. This was made between 1951 and 1972, based on the style of the mark. I think it is the "#505 Bellaire" pattern. I've shown two of these one with the older IG mark and one with a later mark.
A $1 thrift shop find. This is the first slag version of this toothpick holder that I have seen in the wild.
