What model Lightolier lamp is this?

My parents went to the Lightolier store in New York City in January 1965. They were gifted two of these table lamps by the company, and were shipped to our house in Michigan. I can’t find anything on the lamps that indicate who they are made by, but am assuming they are Lightolier, as this was the company that invited my parent to New York, and my Mom remembers the store being the Lightolier store. Thanks for any info you might be able to provide. They are a beautiful blue glass and have been part of our lives for the last 50+ years.
