Looks like title, date, medium (tempera), artist in both English and Polish.

I've had a look at the Polish and transcribed for you it looks like this:
Sw (= Swiety) Jerzy zabijajacy smoka
St George and the dragon
Wiek / -entury 17
Tempera
Wiek (wyk) by W. Witkowski
I've left out the Polish accents. In brackets you have the full form of Saint ( St.).
The last is an alternative to 'wiek'.
I've found what I believe is the original painting or icon on Google images.
The difference about this version of the St George fable are the details; the skull at the bottom left corner.
On the original in the Muzeum Naradowe w Krakowie (National Museum in Krakow) you have the skull and some other figures present as well. Your version is missing a figure of a king to the right of the horse's head. Getting permission to learn more about the icon in the Muzeum Naradowe w Krakowie is difficult: access denied. You have to sign in get password etc. I think that is something you might like to do to find out more about their icon.

But you will be denied access, I think, as was I when I tried to dig deeper.
But from there you will be able to contact the museum. It's bound to be closed at this time of year?
By the way the Polish accents are important If you are looking for the picture in Google images. Without them the image does not appear on Google.
But I have seen it and what interests me is that the work that you have has a reproduction of the original frame as well!
