Looks like a challenge coin that someone put on a chain.

      Size?

The insignia in photo #1 looks like a military beret with flash on the front (in the US associated with ABN or SF units-- and a 10 year stretch +- where everyone wore a black one).

The coat of arms below the beret is El Salvador.

Militarist is right on - The B in shield = El Salvador Army BIRI BELLOSO (GENERAL RAMOS BELLOSO). BIRI = Immediate Reaction Battalion Infantry, trained by US advisors at the School of the Americas.

The "B" has a sword going up through it and 4 lightning bolts surrounding it.

Could be "Por mi patria cualquier cosa, en cualquier lugar." i.e. "For my country anything, anywhere."

...is the inscription on the left image. On the middle image, or back of the medal, it should probably read "Batallon de Infanteria de Reaccion Immediata General Ramon Belloso"

brilliant olofz!!! i composed one side ill try to compose the other, so what i am thinking is that this medal was given to the army trainees by the scholl of the americas after graduation?
