Got it from a friend who told me it is very old jade. Most of it is translucent milky (a little greenish), except for some of the areas with this white rough surface, and some red patch. Can someone tell me what is the white and red spots, and what caused them?

Isn't it just part of the natural colour, you think? If not, what kind of thing is this? The purpose might explain the spots.

Do you have more pictures of the item from different angles? This will help to validate its authenticity. People often confused very old Jade as genuine archaic Jade, but they are mostly made to look old. If your friend is an antique dealer, just be more careful.

The white rough surface could be calcification on the surface while the red patch could be red cinnabar. Don't get too excited yet. The Chinese are experts at falsifying such features, claiming their piece is very old, which cannot fool a trained eye with proper education on Chinese Archaic Jade History.

Just to add, the white and red patches are commonly associated with burial. There are scammers in China who even apply such patches and bury in their backyards. Nowadays, the Chinese loot tomb locations, before informing authorities, and even plant their fakes into the tombs before the authorities and expert arrives.
