I would say it is a hat pin or stick pin. I have a couple from the late 1800's that are cameo and rose designs. They normally have a gold color plug that goes over the pointed end.
It would be a stick pin as it appears to be too short for a hatpin. The cameo is cut in helmet shell. Cameos of this type are fairly common, but I have never seen any two exactly alike. I think of them as country village scenes. Some have no human figures at all, some have highly fantastical buildings, most have wind swept trees. A lone woman is the most common when a person is present in the scene. Sometimes she is feeding a goat or ducks on a stream. Carvers seemed to have used their own imaginations and compositional skills freely.
Sometimes a well or well house is a feature, and these are often mistaken for the Biblical scene of Rebecca at the Well. (Even some with no well get this designation.) Rebecca, plus some non-Rebeccas, is shown here:
