This was part of a PIZZA HUT ad campaign in the late 1980's, shortly after they had introduced their "personal pan pizza" product and were attempting to build their reputation as a restaurant one should visit for a quick, delicious, and affordable lunch. Servers, upon taking your order, would leave this little (about 3" square) timer behind on the table and punch its red button -- it would then count down 5 minutes in which time your lunch was guaranteed to be delivered back to your table or it was free.
[the personal pan pizzas, BTW, were pre-prepared and only took ~2min to cycle (oops...I mean "FRESH BAKE"!) thru the conveyor oven...even a slow server or "make table" employee could usually keep up...I seriously doubt if they 'gave away' as many as they 'threw away', given the prescribed methods for anticipating the daily demand at the time...]
Turns out these were also kinda handy 'kitchen timers' too in a way, ifn's one might have worked at a Pizza Hut back then, maybe finding one in his apron pocket upon getting home from work one night? (or at least that's probably why I still have it...?? <lol>)
This one doesn't actually seem to work anymore, it only displays "1:00" and pushing (or holding) its red button doesn't do anything at all. :-( Maker's info on its back says it was made in China for "ACI INTERNATIONAL".

Funny, I have the same non-functioning timer with the 1:00 display. Found this post while looking for a possible solution to fix it. I have an actual use for a 5 minute timer, too! If anyone has fixed it or knows a parts source, please let me know!
