I found this at Goodwill....but cannot find it anywhere else. It is sandblasted stamped Blenko which dates from 58-61. I'd love to know if someone can verify it and knows the mold/model number. 11 inches high to the highest edge

I did...reached out to a collector and he found it...I didn't see it because per the catalogue, there was no issue in crystal. :)
It is 6010, produced in 1960
