The book is "The Collector's Encyclopedia of Pattern Glass" by Mollie Helen McCain. It is from the late 80's, so it's likely out-of-print. There may be others however. I have only made suggestions....

Looks like it might be a carnival decanter. Check just inside the rim for scratches or wear that may indicate it had a satin/etched stopper.
