Someone is selling one just like it on etsy.com for $29.95. It isn't in as good of shape as yours. It doesn't show the age or date, but maybe the seller would know. Hope this helps.
Samuel Oliver and Mary Ann Lucas are in the Pennsylvania, Church and Town Records, 1708-1985 on ancestry.com, but I don't have access to open it. Hope this helps.
